Box 45 and other information

Required on every T4 since 2023, for the Canadian Dental Care Plan.

Enter the figures from your payroll records. A T4 that misstates remuneration or deductions is a false information return; the employer, not this tool, is responsible for its accuracy and for filing the T4 Summary with the CRA by the last day of February.

What the PDF contains

  • The two employee copies of the T4 for one employee, with the instructions the CRA prints on the back
  • Boxes 14 through 56, including CPP2 (16A), QPP and QPP2 (17, 17A), PPIP (55, 56), the dental code in box 45 and six Other Information codes
  • Contribution boxes checked against the year's ceilings: the $74,600 YMPE, $85,000 YAMPE and $68,900 EI maximum for 2026, with Quebec's rates when box 10 is QC
  • Box 45 required, and boxes 24 and 26 checked against the premiums entered, before the preview renders

What you still file

The CRA copy and the T4 Summary are not in the PDF. File them through the CRA's free Web Forms service or your payroll software; electronic filing is required once you have six or more slips of the same type. Quebec employers also issue the RL-1 through Revenu Québec or their payroll software; with box 10 set to QC, the page shows the Quebec figures to check it against.

Who it is for

A Canadian employer with a few employees who runs payroll by hand or with a pay stub generator and has to issue T4s in February, or an owner-manager who pays themselves a salary and needs one slip a year. The December pay stub's year-to-date totals are the figures the boxes want.

T4 slip questions

Is this an official T4 slip?
The employee copies are customized slips carrying the same boxes and back-of-slip instructions as the CRA's form, which the CRA permits for the copies you give employees. The copy filed with the CRA goes in electronically with the T4 Summary, through Web Forms or your payroll software, not from this PDF.
What does the generator check?
That CPP, CPP2, QPP and EI contributions do not exceed the year's maximums or the rate applied to the earnings you entered, that pensionable and insurable earnings are capped at the year's ceilings, that box 45 is completed and boxes 24 and 26 print 0.00 rather than blank when there are no insurable or pensionable earnings (the CRA reads a blank as box 14), that QPP and PPIP boxes appear only for Quebec employment, and that box 50 is completed whenever box 20 or 52 has an amount. The figures themselves come from your payroll records.
Does it handle Quebec employees?
Yes. Set box 10 to QC and the slip switches to QPP in boxes 17 and 17A, the reduced EI rate in box 18 and PPIP in boxes 55 and 56. The RL-1 the same employee needs for their Quebec return is a separate slip issued through Revenu Québec; the page shows the figures it shares with the T4 so you can check them.
